The Constellation Health Centre is a bilingual primary health care clinic in Whitehorse.
Clinic services
Constellation provides primary health care in a team-based format. This means you have access to a team of licensed health professionals who work together to provide care and support.
The team includes:
- Physicians
- Nurse practitioners
- Registered nurses
- Licensed practical nurses
- Social workers
Primary care services include:
- Diagnosing, ordering and interpreting diagnostic tests
- Basic simple sample collection
- Prescribing and administering pharmaceuticals
- Prescribing and performing specific procedures
- Virtual visits
- Routine check-ups
- Chronic disease management
- Health education and promotion
- Preventative care
- On-site mental health intervention counselling
- Social work support
- Referrals to other health care providers and specialists
Services are provided in both English and French by bilingual staff, with virtual interpretation services available in over 240 other languages, including American Sign Language. Registered clients may access same-day appointments. This means you can call the centre and pre-book an appointment, and if there’s an opening, receive care on the same day.
Who can access care
To access care, you must be a registered client of the clinic. To become a client, complete the application below.
To be eligible for registration, you must:
- have a valid Yukon Health Care Insurance Plan (YHCIP) number; and
- be 16 years of age or older.

How to provide feedback
We invite existing registered clients to provide feedback by following these steps:
- Download or print the client feedback form
- Fill the form with the appropriate information
- Submit the physical copy of the form at the Centre de Santé Constellation Health Centre or email the completed form to, [email protected] Attn: Clinic Manager
Join the Client Advisory Committee
Constellation is seeking registered clients and individuals familiar with the clinic’s services to serve on the Client Advisory Committee.
The committee provides recommendations to help improve Constellation’s primary health care services, including:
- improving quality, access, safety and long-term sustainability;
- supporting monitoring and evaluation activities, and identifying actions based on results; and
- ensuring services reflect language, cultural and gender diversity needs.
